Cancellation Policy

Refund requests must be received in writing by September 7, 2007 and will be minus a 10 percent administrative fee. No refunds will be given after September 7, 2007. Refund requests may be faxed to Jill Annala-Rogers at 602-532-2011 or mailed to: Jill Annala-Rogers, Business Development Coordinator, Arizona Heart Hospital, 1930 East Thomas Road, Phoenix, AZ 85016.

Neither refunds nor credits will be given for failure to attend, late arrival, unattended events, or early departure. Arizona Heart Hospital is not responsible for airfare, hotel, or other costs incurred by participants in the event of program or registration cancellation. In the event that ACT Nurses' Day is abbreviated or cancelled by fire, explosion, strike, freight embargo, epidemic, catastrophe, act of God, or the act of a public enemy, including, but not limited to, an act of any government, de jure or de facto or agency of official thereof, the Arizona Heart Hospital reserves the right, in its sole discretion, to unilaterally terminate the conference. In such cases, the registrant hereby agrees to waive any claim he, she, or it may have against the Arizona Heart Hospital for damages or compensation, including but not limited to fees for registration, accommodation, airfare, and incidental charges.

Special Needs

Arizona Heart Hospital fully complies with the legal requirements of the ADA and the rules and regulations thereof. Any attendee with special needs (e.g., special diet, handicap access, hearing impaired) should indicate this when registering for the conference. Please also notify the hotel when you make your reservation. If you need any kind of auxiliary aid, please indicate this when you register for the conference and make your hotel reservation so that such arrangements can be made on your behalf.
